Welcome to UCLA! If you are interested in playing a club sport at UCLA this year, you will be required to submit a pre-participation physical as part of the registration process.
The UCLA Arthur Ashe Student Health and Wellness Center provides medical oversight for the UCLA Club Sports Program and collaborates with UCLA Recreation to support athlete health and participation. Services include athletic training services, pre-participation physical evaluations (PPEs), sports medicine care, medical clearance review, and coordination of specialty referrals.
Due to high demand for PPE appointments during Fall Quarter, athletes are encouraged to complete their PPE with their primary care provider before arriving on campus whenever possible.

Athletes who are planning on completing their PPEs at Ashe will only need to complete the electronic questionnaire through their Ashe MyStudentChart portal (MyChart - Login Page). Athletes who plan to complete their PPE outside of The Ashe Center should print out the form and complete the two-page questionnaire prior to their appointment. The clinician will then complete the form.
