UCLA Club Sports

Welcome to UCLA! If you are interested in playing a club sport at UCLA this year, you will be required to submit a pre-participation physical as part of the registration process.

UCLA Recreation and Club Sports works closely with the UCLA Arthur Ashe Student Health and Wellness Center to provide medical services and referrals for club sport athletes. However, appointments at the Ashe Center are in high demand in Fall. we encourage you to complete the PPE at home with your primary care provider prior to coming to campus. Athletes who are planning on completing their PPEs at Ashe will only need to complete the electronic questionnaire through their Ashe student portal. Athletes who plan to complete their PPE outside of the Ashe Center should print out the form and complete the two-page questionnaire prior to their appointment. The clinician will then complete the form.
